Vasko will pay the first $350 of premium cost per month. The employee pays the difference through pre-tax payroll deduction. Here is the current premium schedule. Health insurance coverage begins the first day following the completion of a 90-day probationary period.

BlueCross BlueShield Coverage
  • 100% coverage for routine preventative care.
  • 100% coverage for office visits due to illness or injury after a $15 copay.
  • 100% coverage for emergency facility fee after $60 copay.
  • 80% coverage for emergency physician sevices.
  • 80% coverage for inpatient hospital and physician services when provided by Blue Choice network providers.

  • Extended network and out-of-network services are covered at a reduced benefit.
  • $3,000,000 lifetime maximum.
  • Coverage is extended beyond age 19 to age 25 for those unmarried dependents attending school on a full time basis.
  • The providers in the Blue Choice network must be used for all services to be paid at the highest coverage level including chiropractic, physical and occupational therapy. No coverage when using nonparticipating providers.
  • Prescription drugs have a $14 copay for formulary drugs; no coverage for nonformulary drugs.
  • In addition to health coverage, you also have access to health improvment programs designed to help you feel better about your health. Great "extras" at no additional cost! Below is a breif summary:
    • BluePrint for health programs include 24-hour nurse advice line, card support program, complementary health card network and products. BluePrint for Health on-line at bluecrossmn.com, stop smoking program and Healthy Start prenatal care program.

Vasko offers dental insurance through Delta Dental at an employee cost of $6.50 per pay period for families (2 or more), and $2.25 per pay period for single coverage. Vasko covers the rest of the premium expense. Dental premiums are a pre-tax payroll deduction.
Here is a summary of the Vasko's dental benefits
Delta Dental pays the following percentages of the treatment cost up to a total of $300 per year. The maximum fee payable by Delta for each procedure is determined by Delta.
  • Diagnostic and preventative services 100%
    • This includes oral examinations at 6-month intervals and bitewing x-rays at 12- month intervals
  • Basic services 50%
  • Endodontics 50%
  • Periodontics 50%
  • Oral surgery 50%
  • Major restorative services 50%

There is no deductable on this plan.
The coverage year is January 1 to January 1.

 

This plan allows you to pay medical, dental, adult and child care expenses with "untaxed" dollars. Each dollar allocated to this plan saves the average employee from $0.30 to $0.40.

Current employees can enter the plan on January 1 and new employees may enter the plan after the first 90 days of employment.


Here's how the plan works
  • Each employee determines how much money they want redirected from their paycheck each week to pay for eligible expenses
  • When you pay for eligible expenses simply submit the receipt and you will be reimbursed.
  • Note that all of the money allocated to the plan must be spent or it will be lost. You must estimate your expenses conservatively.
  • Examples of some allowable expenses are: health care deductibles, co-pays, dental bills, eye care expenses, day care expenses, tuition fees for pre school. For a complete listing contact Freedom Services.

401K Retirement Plan

Vasko employees are eligible to enroll in this program each January 1. Vasko will contribute 50¢ on each $1 an employee puts in their account up to 6% of your salary.
  • Rollovers from other 401K programs are allowed at any time.
  • New enrollments are allowed only on January 1 of each year.
  • Once you are enrolled and have participated for at least one month, you may start or stop your participation on the first of any month.

Vacation and Holiday Policies

 
  • Employees who are eligible to receive one week of vacation, must take the time off with pay.
  • Employees who are eligible to receive two weeks of vacation, must take both weeks off with pay.
  • Employees who are eligible to receive three weeks of vacation, must take two weeks off and can receive pay for the third week if they desire.

Vacation Carry Over

  • After 1 year of service, no vacation carry over
  • After 2 years of service, 3 days can be carried over
  • After 5 years or more of service, 1 week of vacation can be carried over

Vacation Request Policy

  • Vacation reservations are on a first come first serve basis at this time.
  • Drivers, If a driver has 3 weeks or more of vacation, only 2 weeks can be taken between April and October.
  • To request vacation, fill out a "Time Off Request" sheet. Until this sheet is signed by your supervisor, do not plan that you will have the time off. Do not make reservations or buy tickets without a signed "Time Off Request" sheet.

Holidays (Full Time Employees)

  • To receive holiday pay you must: report to work the day before the holiday and the day after, or have a vacation day scheduled, day off without pay scheduled, or a doctors excuse.
  • Holiday work may be required at management's discretion.
  • Holiday pay is 8 hours straight time. This is the same for employees that work 5 days per week or employees that work 4, 10-hour days per week. Any hours worked on a holiday go towards the 40 hour total which must be reached before being paid overtime.

Christmas Savings Plan

  • You can have an elected dollar amount deducted each pay period from the first pay period in January until the Christmas savings plan pay out on the first pay period in November.
  • If you elect this plan and emergency arises that you must take an early payout, it will be paid out the following payroll after Vasko is notified and you must remain out of the plan for the remainder of the calendar year.
